All the documents should be in 2 Photocopies (self signed ) along with Originals at the time of Submission of  Application Form.

Photographs

• 6 passport size identical recent, click color photographs whit  background

• Size-35mm x 35 mm showing frontal view of full face

• Photos taken with Colored or Dark Glasses & in Uniform will not be accepted

Date Of Birth Proof (Any TWO )

• Birth Certificate corporation

• School Leaving Certificate

• Board Certificate

• Bonafied  (For Child 5 to 18 years)

• Corporation Certificate (For Child 5 to 18 years)                                              

• Birth after 1988 Corporation Birth Certificate

Passport Emigration not required papers ( ECNR) ( ANY One)

• 10 ^th Board Certificate or 12 ^th Board Certificate

• Degree with Mark list

• Income Tax Papers of last 3 years (saral)

Address Proof ( any two)

• Ration card or Bank Pass Book

• Electric Bill & Telephone Bill, Personal Name (One recent & One 1/1/2 months old)

After Marriage

• Marriage certificate OR Marriage Affidavit. (If first name is change then make Affidavit for passport office Performa)   

Photo ID Compulsary

Note:   All the documents are required will be Original with Two Photocopies
